即將開始外來人口變更統一證號申請流程，請備妥您的新式統一證號證件「居留證」或「中華民國統一證號基資表及護照」，為保障您的權益，請詳細閱讀以下內容：
The application for changing the new UI No. for foreigners is about to start. Please prepare your new UI No. certificate, the Alien Permanent Resident Certificate (ARC) or UI No. Basic Information Form of R.O.C. and the passport.
本人已詳閱上述約定事項之內容，並同意申請辦理變更新式統一證號（含以本人為負責人/代表人之非個人客戶基本資料所涉本人統一證號資料)。 I have read the contents above carefully and agree to apply for the change and update of my new UI No. (including the information of non-individual client whose responsible person/representative is the Applicant)
本人已詳閱『兆豐國際商業銀行履行個人資料保護法第八條第一項告知義務 』、『兆豐國際商業銀行履行個人資料保護法第九條第一項告知義務 』及『兆豐國際商業銀行告知書附表 』所載相關內容，並同意貴行/財團法人金融聯合徵信中心(下稱聯徵中心)於該內容所載特定目的範圍/聯徵中心營業登記項目或章程所定業務需要等範圍內，得蒐集、處理及利用本人個人資料(包括統一證號、中英文姓名、出生日期、居留期限、居留地址、性別、國籍、護照號碼、肖像、聯絡電話等)。 I have read the relevant contents specified in the Mega International Commercial Bank Informing the Execution of Personal Information Protection Act, Article 8 Section 1, Mega International Commercial Bank Informing the Execution of Personal Data Protection Act, Article 9 Section 1, and Attached list of Notification for Collection, Processing and Use of Personal Data by Mega International Commercial Bank, and agree that the bank or the Joint Credit Information Center (hereinafter referred to as the Center) may collect, process, and utilize my personal data within the scope of the specific purposes specified in said contents or within the Center’s business scope registered or business scope specified in its articles of incorporation (including the new UI No., Chinese and English names, date of birth, expiry date of residency, residence address, gender, nationality, passport number, portrait, telephone number, etc.)
